This application must be filled out completely or it will not be considered for an adoption. This is not to invade your privacy, but is for the protection of the chinchilla which is our top priority. All information is strictly for the purpose of conducting an adoption screening and is used only by Forever Feisty Chinchilla Rescue Inc. We have the right to refuse any applicant if we feel it is not in the best interest of the chinchilla. All chinchillas have had a complete physical exam by a veterinarian before adoption. The adoption fee is $75.00 per chinchilla; payable by cash, money order or cashiers check only. All fees will go into the rescue's funds for the chinchilla’s healthcare & daily living expenses.

How did you hear about our rescue?

You agree to provide a proper & healthy chinchilla diet, fresh water, appropriate indoor shelter and medical care for the adopted chinchilla(s). You also agree that the chinchilla(s) will be under the care of the primary person named on this application and reside at the residence which is also listed for said applicant. All forms of housing intended for the adopted chinchilla(s) must be approved by us before any adoption may be completed. Adoption fee is due at time of completed adoption with new guardians.

The primary person whose name is listed on this application must be at least 18 years of age. Granting permission for our rescue to check all references listed is part of the agreement by you when applying for a chinchilla adoption. By submitting this application you certify that all the information enclosed on this application is true. You also agree that if for ANY reason you no longer wish to care for the chinchilla(s) adopted through the Forever Feisty Chinchilla Rescue Inc., you will surrender the chinchilla(s) back to the rescue with no questions asked. You will NOT under any circumstances sell, give away, or discard into the wild the above mentioned chinchilla(s).
